Home Health Physical Therapist - Zachary, LA (FLEXIBILE SCHEDULE)

Renowned & award-winning patient-oriented Home Health Physical Therapy Healthcare organization currently has openings needed for experienced, dependable Physical Therapists (PT) and Physical Therapist Assistants (PTA) to work with assigned patients in their homes in Zachary, LA to provide care based on specific Treatment Plans provided by hospital. Same assigned patients each week! Will schedule PT & PTA's shifts based on availability, and need of Patient Home Health Physical Therapist hourly needs per week

Join on within a new program starting out for hospital to provide more services to the community

W2 | FLEXIBILE SCHEDULE: Able to work Full-time (Mon to Fri, 40 hr/wk), Part-time (20 hours/week), or PRN
  • Mon to Fri, 8a –5p (FT 40 hours/week)
  • OR: as many days/week as needed
PAYRANGE: $54.18/hr - $57/hr (based on experience)
  • PLUS: *BONUS: $250 Monthly Bonus given each month to cover mileage - does not expire
  • Starts with IMMEDIATE BENEFITS: Monthly Bonus ($250), Medical, Dental, Vision, Supplemental Insurance (@ no-cost)

Home Health Physical Therapist - JOB SUMMARY:
  1. Initial Patient Evaluations at home
  2. Set up Treatment Plan
  3. Perform Pain Relief and Management
  4. Injury Rehabilitation and Recovery
  5. Improved Mobility and Function
  6. Management of Chronic Conditions
  7. Prevention and Wellness
  8. Enhanced Physical Performance
  9. PTAs: For patients who receive care through Medicare, Medicaid, Private insurance or Private pay programs

REQUIREMENTS:
  • Physical Therapy Certification and program completed
  • 1+ year Physical Therapist experience
  • 1+ year of experience working in Home Health 
  • Reliable transportation, auto insurance
  • Ability to commute to patient homes within vicinity of Zachary, LA
  • Specialty experience preferred - Geriatrics, Post-op, Rehabilitation, Home Health, Orthopedics, Physical & Rehabilitation Medicine, Sports Medicine, etc.
